DOA-733 <br />(Rev.1/00) <br /> <br />Home and Community Care Block Grant for Older Adults <br /> County Funding Plan <br /> <br />Methodology to Address Service Nccds of Low-income Minority Elderly <br />Older Americans Act, Section 306 (a) (5) (A) (ii) <br /> <br />~ommunity Service Prcvlcer Cabarrus County Department of Aging <br /> <br />County Cabarrus July 1, 2001 through June 30, 2002 <br /> <br />The Older Americans Act requires that the service p. rovider attempt to provide services to low-inC°me minority <br />individuals In accordance to their need for aging services. The community service provider shall specify how the <br />service needs of Iow-income minority elderly will be met through the services Identified on the Provider Services <br />Summary (DOA-732). This narrative Shall address outreach and service deliuery methodologies that will ensure <br />that this target population is adequately served and conform with specific objectives established by the <br />Area Agency on Aging, for providing services to Iow income minodty Individuals. Additional pages may be used <br />as necessary. <br />GOAL: <br />To ensure the participation in services provided through the Cabarrus County Department of Aging of the Iow <br />income minority elderly in the community. <br /> <br />The Cabarrus County Department of Aging provides services to all older adult residents regardless of race or <br />economic status. Brochures are distributed throughout the community that promote the services provided by our <br />agency. Services are also promoted through local churches and through speaking presentations to senior <br />citizen and local civic groups, as well as the local Parish Nurse Programs. <br /> <br />Our current client base includes a substantial number of minority clients and some of our Lunch Plus Club sites <br />are located In minority and rural areas. Transportation is provided to all Lunch Plus Club programs. We will <br />continue to distribute Information in predominately minority and/or Iow income neighborhoods, promote our <br />services through local churches, make personal presentations to groups, and place informational brochures <br />in local medical offices/facilities. <br /> <br />Our local newspaper, The Independent Tdbune, prints and distributes our Sunset Outlook newsletter bi-monthly <br />to 27,000 subscribers. A second newsletter (Senior Savvy) is distributed monthly. This publication is distributed <br />on counter tops throughout the rural areas of Cabarrus county. We also have started producing a twice weekly <br />Television Show (Forever Young) that addresses Aging issues and is available on local Cable TV. <br />This allows publicity of our programs and services to reach the entire county. We also coordinate service <br />referrals with other human service agencies and receive referrals for services from various local service <br />~rovlders. <br /> <br /> <br />
