The community <br /> service provider shall specify how the service needs of low-income minority elderly and rural <br /> elderly will be met through the services identified on the Provider-Services Snmm~ry CDOA-732). <br /> The narrative sh~ll address outreach and service delivery methodologies tha~ wr2l ensure that thio <br />· target pop~]lation is adequately served and confo.m with specific objectives established by the <br />Area Agency on Ag/ng, for providing services to low-income minority individuals. Additional <br />pa~es may be used as necessary. <br /> <br />GOAL: To increase participation in sea, rices and programq.by low-income minority elderly · <br />persons at a minlmnm rate of~e percentage of Iow-income minority elderly in the commullity. <br /> <br /> Cabarrus County Department of Social Services provides assistance'to ali individuals <br />requesting services regard/ess of race with most of our programs serving logy income indSviduaks. <br />The type or exteut of services clients receive are based solely on clients' need and ava~qslx~ty of <br /> <br /> Through the HCa3G Program, 26% of the clients served are in the minority populatio~ ' <br />Th~s exceeds the 13.8% minority population of Cabarrus County which is reflected in the 1990 <br />census statistics, We will continue to stt~ve to serve the low-income mlnorlty population In our <br />Community Alternatives Program (CAP), 35% of the clients served are in ~hls group; In additiorg <br />32% of the clients served through the In-Home Aide Program had income under 100% of the <br />Federal Poverty Level_ <br /> <br /> Outreach by the agency b~.q been accomplished by newspaper articles, brochurea~' home <br />v/sits by social work st~ff, and contacts with other agencies. Since funding ~s l~mited for these <br />programs, any new outreach programs vn2l not implement; however, current 'outreach efforts, <br />which have proven effect/ye in the pa2st, will continue. Based orr current need for serv/ces, there <br />are hot suffi~ funds for a significant increase,in the n~rmber of clients served. Requests ~om <br />clients who meet the need for In-Home. Aide Ser~ces w~l continue to be accepted. Blew clients <br />will be added as ex/sting clients leave the prograka or experience a decrease in the number ofrm~ts <br />needed or additional funding ks ~ec~red. <br /> <br /> A~ an agency with funding from several sources, assessments wflI be made with each case <br />~o be sure thst f~cting is used to best meet the needs of the clients and that Older Americans Act <br />fuuds are targeted towards low-income minority indi~duals. <br /> <br /> <br />
