Applications for a fra~-hi~e to operate Ambu- lances in the county pursuant to ~hiA article shall be made upon such forms as may be prepared or prescribed by the county and shall con~in: (1) The type of franchise requested, either nonemergency Ambulance service or emer- gency Ambulance service. (2) The name and address of the applicant. (3) The name under which the applicant does business er proposes to do business along with a carei6ed copy of any articles of ~i~cate of limited partnership, or as- s~,med n,rn~ care/6cate. (4) The ~r=inlng and experience of the appli- cant'in the transportat/on and care of patients. (5) A description of and 'copy of the state certification for each Ambulance operated by the applic~-t. (6) A copy of the state certification for each emergency reed/cai technician employed by the applicant.. (7) The lecat/on and description of the place from wh/ch it ia intended to operate. (8) Audited or unaudited financial state- ments of the 'owners operations in the county, in such fo,~ and in such detail as may be required by the county. (9) Any .other i~formation the county shall deem reasonably necessary for a fair de- termination of the capability of the appli- cant to pro~ide A,~bulance service in the county in accordance with the require- ments of state law and the provisions of this article. (Ord. of 9-!-90, § 3) Sec. 82-53. Hearing, investigation, .standards for g~.~ ntlng. (a) Upon receipt'of an application.for a fran- ehlse, the board shall schedule a time and place for a hearing on ~he,~a. pplicatien. (b) The county sh~l!; prior to the hearing, cause such invesSgation as is deemed necessary to be made of the applicant and its proposed operations. (c) A franchise may' be granted if the board ~nds ail of the following: (1) The public convenience and necessity re- quires the proposed ~mbulance service. (2) Each such -mbul~,,~e and its required equipment have been certifie~lby the state as acceptatrle-'for the type of fran~hi.,_e (3) The applicant or its ~cers. or partners are respo,~le persons to conduct or work in the proposed business. (4) Only duly certified emergency medical technicians are employed as operators and attendants by applicants for an -tabu- lance service fran~-hi.,e.
