Application. <br /> <br /> Applications for a fra~-hi~e to operate Ambu- <br />lances in the county pursuant to ~hiA article shall <br />be made upon such forms as may be prepared or <br />prescribed by the county and shall con~in: <br /> <br /> (1) The type of franchise requested, either <br /> nonemergency Ambulance service or emer- <br /> gency Ambulance service. <br /> <br /> (2) The name and address of the applicant. <br /> (3) The name under which the applicant does <br /> business er proposes to do business along <br /> with a carei6ed copy of any articles of <br /> <br /> ~i~cate of limited partnership, or as- <br /> s~,med n,rn~ care/6cate. <br /> (4) The ~r=inlng and experience of the appli- <br /> cant'in the transportat/on and care of <br /> patients. <br /> <br /> (5) A description of and 'copy of the state <br /> certification for each Ambulance operated <br /> by the applic~-t. <br /> (6) A copy of the state certification for each <br /> emergency reed/cai technician employed <br /> by the applicant.. <br /> <br /> (7) The lecat/on and description of the place <br /> from wh/ch it ia intended to operate. <br /> <br /> (8) Audited or unaudited financial state- <br /> ments of the 'owners operations in the <br /> county, in such fo,~ and in such detail as <br /> may be required by the county. <br /> <br /> (9) Any .other i~formation the county shall <br /> deem reasonably necessary for a fair de- <br /> termination of the capability of the appli- <br /> cant to pro~ide A,~bulance service in the <br /> county in accordance with the require- <br /> ments of state law and the provisions of <br /> this article. <br />(Ord. of 9-!-90, § 3) <br /> <br />Sec. 82-53. Hearing, investigation, .standards <br /> for g~.~ ntlng. <br /> <br /> (a) Upon receipt'of an application.for a fran- <br />ehlse, the board shall schedule a time and place <br />for a hearing on ~he,~a. pplicatien. <br /> (b) The county sh~l!; prior to the hearing, <br />cause such invesSgation as is deemed necessary <br />to be made of the applicant and its proposed <br />operations. <br /> (c) A franchise may' be granted if the board <br />~nds ail of the following: <br /> <br /> (1) The public convenience and necessity re- <br /> quires the proposed ~mbulance service. <br /> <br /> (2) Each such -mbul~,,~e and its required <br /> equipment have been certifie~lby the state <br /> as acceptatrle-'for the type of fran~hi.,_e <br /> <br /> (3) The applicant or its ~cers. or partners <br /> are respo,~le persons to conduct or work <br /> in the proposed business. <br /> (4) Only duly certified emergency medical <br /> technicians are employed as operators and <br /> attendants by applicants for an -tabu- <br /> lance service fran~-hi.,e. <br /> (5) That all the requirements of this article <br /> and all other applicable laws and ordi- <br /> nances have been met. <br />(Ord. of 9-4-90, § 4; Ord.
