PART III (of 3 Parts) <br /> <br />A.) Weatherization Assistance Program. The following shall appty to the Weathedzation <br />Assistance Program as noted in Part I, Section 4A: <br /> <br /> SectiOn 1' Pro_9.g_~m Guidance. The Subgrantee shall provide to the Subcontractor <br />copies of all rules and regulations, instructions, and other documents pertaining to the <br />Program. In no instance shall the Subcontractor take action on any rule, regulation, or <br />instructive printed in the Federal Register or other document, without first notifying the <br />Subgrantee of such intention. In the event that rules, regulations, or procedures are revised <br />or amended, the Subgrantee will forward such revisions or amendments to the Subcontractor <br />and thereby make them a part of this Agreement. <br /> <br /> Section 2: Certification of Applicants. The Subcontractor shai! give priority to <br />identifying and providing assistance under this~ Program to elderly and handicapped <br />Iow-income persons and to provide assistance to those persons whose income is at or below <br />150% of the poverty level as determined by the Office of Management and Budget, or who <br />received cash assistance payments under 'r'~de IV or XVI of the Social Security Act or other <br />applicable State or local law. Applicants must have received assistance during the preceding <br />12 months in which weatherization funds are being appropriated. The Subcontractor 'must <br />certify the eligibility of each household by completing the DOE Homeowner Cert~cation <br />Form, EIA-29D. Eligibility shall be certified no less than six (6) months prior to receipt of <br />weatherization assistance. In those instances where dwellings are not owned by the <br />applicant, the Subcontractor shall execute an Agreement for Participation for Leased <br />Dwellings. The Subcontractor must also obtain a Release Work Order from the applicant <br />prior to the actual weatherization activities. <br /> <br /> Section 3: Prioritization of Client Categories. The Subcontractor shall give priority to <br />identifying and providing assistance under this Program to elderly and handicapped - <br />Iow-income persons. The Grantee has established statewide completion goals of 39% <br />elderly, 14% handicapped and 47% other. In addition, the Grantee has established a <br />statewide completion goal of 31% rental units. The Subcontractor shall, to the maximum -- <br />extent practicable, attempt to meet these percentage goals. <br /> <br /> Section 4: The Subcontractor shall not exceed an avera~je cost of $2,085 per - <br />dwelling unit completed (not including administrative costs) nor shall it exceed $1,250 <br />materials cost on any individual unit completed. Where it is determined that the average unit <br />cost or the individual unit materials cost is insufficient to meet client needs, the Subcontractor - <br />shall submit a written request for a Materials Cost Waiver to the Subgrantee who will in return <br />request a waiver from the Grantee for approval. <br /> <br /> Section 5: $.chedule of Iml31ementation. The Subco,[[actor agrees thata minimum of <br />25 dwelling units'~a-e to be~ea~herized during the grant period defined in Part 1, Section 3. <br /> <br />In addition, the. Subcontractor agrees that if the rate of completion of weatherized units fails to <br />a level that will prevent the Subcontractor from utilizing the full amount of funds provided <br />under this co~'uact, then the Subcontractor will notify the Subgrantee of such a situation and <br />allow the renegotiation of the contract to a funding level consistent with the Subcontractors <br />~[~ent rate of production. In the event that the Subgrantee receives an approved.Average <br />Cost Waiver from the Grantee, the minimum unit requirement shall be adjusted accordingly. <br /> <br /> <br />
