SECTION IV: REPORTING AND COMPLIANCE Co Provider shall provide the appropriate representative with the necessary credentials to attend all client staffmgs, court sessions as necessary, senior advisory committee meetings, conferences and training seminars. Provider shall be required to provide on a monthly basis, client specific infom~ation to be utilized by the court to monitor treatment compliance and produce reliable outcome measures. The provider shall collect at a minimum the following data: Partial List of Data to be Collected: 1. Number of clients assessed 2. Number of clients entering treatment. · 3. Number of clients obtaining each level of treatment and the number of hours of treatment per client. 4: .Number of clients referred to alternative treatment programs. 5. Number of clients completing treatment and/or each phase of treatment. 6. Number of clients exiting treatment and means/reasons for tem~ination. Provider shall comply with all applicable provisions of the Criminal Justice Partnersh/p Act and complete and submit all necessary forms and/or reports required. Provider shall make all treatment documentation/files available for review as necessary. The services delivered by the Provider under the terms of this conlsact shall meet the minimum standards of providing substance abuse treatment services set forth by the North Carolina Depat:tment of Health and Human Services (DHHS). Provider wilt comply With the appropriate inspection, licensing, and zoning required by the State Fire Marshall's Office, as well as other applicable local regulations. Provider shall abide by ali laws and regulations of the State of North Carolina, and shall hotd appropriate licenses necessary to provide substance abuse treatment. H.
