Cabarrus County Department of Aging <br />Summary of New Positions <br />September 2001 <br /> <br />Caregiver Support Specialist (Salary Grade 63) <br /> <br />· New 20 hour per week part-time position; <br />· Funded through Federal/State funds with a 25% local match; <br />· Coordinate training programs, services and/or assist with access to services for <br /> caregivers; <br />· Work with the regional (COG) Caregiver Specialist and the state (NC Division of <br /> Aging ) Caregiver Specialist to meet the requirements of the standards regarding local <br /> caregiver services; <br />· Coordinate with and monitor Caregiver services provided by the Coitrane Life <br /> Center. <br /> <br />Aging Wellness Coordinator (Salary Grade 64) <br /> <br />· New full time position; <br />· Funded through a collaborative partnership with the Cabarrus Health Alliance, <br /> NorthEast Medical Center and the Community Free Clinic (Kate B. Reynolds <br /> Charitable Trust) ~for a five-year period: <br /> $7,000 required match is to-be paid with funding from the NC Division of <br /> Aging - Senior Center General Purpose funding; <br />· Work with the Health Alliance Wellness Coordinator to implement a county wide <br /> wellness program to target at risk individuals. This employee will provide aging <br /> related services to provide smoking cessation, nutrition/diet education and <br /> fitness/exercise education; <br />· Program development and training will be provided by NorthEast Medical Center and <br /> the Cabarms Health Alliance will serve as the lead agency for this project; <br />· The project is being supervised by Duke University as a facilitator for this Kate B. <br /> Reynolds project. <br /> <br /> <br />
