STATEMENT OF PROBLEM: In concise terminology, describe the juvenile problem the program <br />intends to ~&~e~s. <br /> <br />Many families in Cai:arms County are experiencing significant life stressors that are minting imo <br />significant family problems. Many of these are single-lmrem families or blended families. Parent~ may <br />exhibit a lack of [~renting skills and children may not receive proper supervision because of parent's work <br />shifts. There may be increased drug and alcohol abuse by the Imrents and/or chil~eat as well as increased <br />school difficulties by the child. Within this type of environment youth may develop socially i .narspr~ <br />behaviors and cr/minal behaviors which result in involvement and could result in out of home placement ia <br />a training school, psychiatric hospilal, child caring institution, group home, or foster home. It apgears that <br />some of these delinquem and undisciplined behaviors reflect the structural and relationship premieres that <br />are occurring with the youth's family unit. <br /> <br />PROGRAM GOAL: State the effec~ that the program is designed to have on solving the probh~m <br />deso~d abova <br /> <br />The overall goal of Home-Based Services is to help keep families together while, 1) a_&~isting f~ed <br />youth in changing theft' delinquent and p~¢-delinqucnt behaviors, and, 2) assi_~ng familie$ in learning how <br />to effectively manage behaviors and issues that are affl~'ting the bchavint$. <br /> <br />TARGET POPULATION: Deso/be the torg~ popula~n ~nd the steps that the program has taken to <br />/nsatre that the targetpopu/at/on is ~ <br /> <br />The pol~,lat/on served by thin program is those youth adjudicated deli .m'!ucnt and/or undisd~ined and <br />youth-at-risk. Referrals to the program are made by District Court Judges and Juvenile Cou~ Counselors. <br /> <br />3ffEASURABLE OBJECTIVE(S): ~t__~__ in me__asurable terms the intended effect of the program on <br />specific #~ed a~d/or delinquent behavior& ,4t a minimum, ~at,, anticipated in court referrals, <br />runaway behavior, and school suspensions and expulsions. <br /> <br />4. <br />$. <br />6. <br /> <br />Continue to develop a service plan for 100% of the children ia tl~ program <br />Rednco out-of-home placements so that 87% of the youth served by thin program during the year <br />will be in the home at the end of treo__tmenL <br />Continue to rech~ youth involvement in the juvenile justice system by 85%. <br />Continue to increase school attendance for youth. <br />Decrease school discipline problems while the youth is receiving services in the program. <br />Continue to rech~ce recidivism back into the juvenile justice system for 65% of youth following <br />termination with the service (Promotion of family self-sufficiency). <br /> <br />Information Maintained for Effectiveness Measurement: List the data elements and records the <br />program will maintain to document its effectiveness. <br /> <br />The program mnlntain~ b*~dgetary information regarding revenues and expenditures of JCPC funds. <br />Documentation of staff time involved ia the program is mairltained through the ~eat of Social <br />Services lnformnhon Sy~em Client files contain the following information: social history fxom referring <br />agency, referral form, aoptication for services, services plan, and case dictation. JCPC client tracking <br />forms are also contained in client file and are submitted to the JCF'C office. <br /> <br /> <br />
