PART III (of 3 Parts) <br /> <br />A.) Weatherization Assistance Program. The following shale apply to the Weatherization <br />Assistance Program as noted in Part I, Section 4A: <br /> <br /> Section 1: Program Guidance. The Subgrantee shatl provide to the Subcontractor <br />copies of all rules and regulations, instructions, and other documents pertaining to the <br />Program. In no instance shall the Subcontractor take action on any rule, regulation, or <br />instructive printed in the Federal Register or other document, without first notifying the <br />Subgrantee of such intention. In the event that rules, regulations, or procedures are revised <br />or amended, the Subgrantee will forward such revisions or amendments to the Subcontractor <br />and thereby make them a part of this Agreement. <br /> <br /> Se~on 2: Certification of Applicants. The Subcontractor shall give priority to <br />identifying and providing. assist~'~ce under this Program to .elderly and handicapped <br />Iow-income persons and to provide assistance to those persons whose income is at or below <br />150% of the poverty level as determined by the Office of Management and Budget, or who <br />received cash assistance payments under 'rifle IV or XVl of the Social Security Act or other <br />applicable State or local law. Applicants must have received assistance during the preceding <br />12 months in which weatherization funds are being appropriated. The Subcontractor must <br />certify the eligibility of each household by completing the DOE Homeowner Certification <br />Form, EIA-29D. Eligibility shall be certified no less than six (IS) months prior to receipt of <br />weatherization assistance. In those instances where dwellings are not owned by the <br />applicant, the Sub,contractor shall execute an Agreement for Participation for Leased <br />Dwellings. The SUbcontractor must also obtain a Release Work Order from the applicant <br />prior to the actual weatherization activities. <br /> <br /> Section 3: Prioritization of Client C.ate~jories. The Subcontractor shall give priority to <br />identifying and pr~/iding assistance under this Program to eldedy and handicapped <br />Iow-income persons. The Grantee has established statewide completion goals of 39% <br />elderly, 14% handicapped and 47% other. In addition, the Grantee has established a <br />statewide completion goal of 31% rental units. The Subcontractor shall, to the maximum <br />extent practicable, attempt to meet these percentage goals. <br /> <br /> Section 4: The Subcontractor shall not exceed an avera~je cost of $2,085 per <br />dwelling unit completed (not including administrative costs) nor shall it exceed $1,250 <br />materials cost on any individual unit completed. Where it Is determined that the average unit <br />cost or the individual unit materials cost is insufficient to meet client needs, the Subcontractor <br />shall submit a written request for a Materials Cost Waiver to the Subgrantee who will in return <br />request a waiver from the Grantee for approval. <br /> <br />Section 5: Schedule of Impla. mentation. The Subcontractor agrees thata minimum of <br />/6 dwelling units'are to ~e~ea~heri~.ed' during the grant period defined in Part I, Section 3. <br />In addition, the. Subcontractor agrees that ff the rate of completion of weatherized units fails to <br />a level that will prevent the Subcontractor from utilizing the f~ll amount of funds provided <br />under this contract, then the Subcontractor will not~y the Subgrantee of such a s'~uation and <br />allow the renegotiation of the contract to a funding level consistent with the Subcontractor's <br />current rate of production. In the event that the Subgrantee receives an approved.Average <br />Cost Waiver from the Grantee, the minimum unit requirement shall be adiusted accordingly. <br /> <br /> <br />
