Identify the staff and their responsibilities to the program, (2) describe the physical facilities in which the
services will be delivered, (3) discuss any areas of concern that could affect the success of the prografn, (4} discuss why these
resources used in the manner described should be expected tq alter inappropriate behavior, (5) describe the referral, screening
and admission procedures, (6.) describe the means of working with youth and familt'es, (7) describe the daily program activities,
(8} describe the termination procedures and (9) interaction with juvenile court.

Five nurses will be assigned to the five middle school campuses in the county. They would be supervised
by a school nursing supervisor from the Cabarrus Health Alliance. The nurse will collaborate with school
personnel (principal, guidance counselors, teachers, social workers) and other community agencies to
target high-risk youths who have been identified as "at risk" adolescents. Through one-to-one interaction
on campus on a daily basis and in the community with the family the nurse will work with the teen to
reduce high-risks behaviors and remain in school and out of the court systems.

Referrals to the nurse may come from the school staffas well as agencies outside of the school. The nurse
will serve as the 'clearinghouse' for the "at risk" student and be his/her daily "in school" support person.
The nurse will user her knowledge of community agencies and resources to assist the youth and his
family with an array of identified needs. When a referral is made to the nurse, she will follow-up with the
youth and/or the family until resolution to the referral is complete.

-4-
