Adult Day Care Agreement <br />Loving Years Adult Day Health Center, Inc. <br />Page 2 of 4 <br /> <br />conduct assessment site visits on the Contractor as necessary to insure compliance with <br />HCCBG standards and collect necessary data to provide accurate documentation to the AAA <br />during the annual County "Provider Review" assessment. <br /> <br />8. provide technical assistance to the Contractor as needed. <br /> <br />II: RESPONSIBILITY OF CONTRACTOR: <br /> <br />The Contractor shall: <br /> <br />1. be a "direct service provider" for any eligible Cabarrus County resident(s) that choose <br /> to receive Adult Day Care Services through HCCBG funding from the Contractor. <br /> <br />2. make available to the County and/or the AAA any/all records relating to clients that receive <br /> Adult Day Care Services fi:om the Contractor through HCCBG funding. <br /> <br />provide the Connty with a copy of a Certificate of Insurance that will verify adequate current <br />insurance coverage. Furthermore, the Contractor agrees to maintain in effect full insurance <br />coverage, in amounts no less than the industry standard. <br /> <br />4. maintain appropriate Certification by the State of North Carolina to operate an Adult Day <br /> Care program for each facility. <br /> <br />notify the County immediately ( same day ) in the event of any adverse action regarding <br />Certification that may place the Adult Day Care Services program in a provisional, <br />revocation or terminated status of operation. The Contractor will continue to keep the County <br />updated and informed concerning corrective action utilized to address deficiencies with <br />complete corrections made within the specified time frame to the State of North Carolina <br />standards. <br /> <br />secure at no expense to the County, all required personnel and appropriate professional <br />training to meet the licensing requirements necessary to operate this Adult Day Care <br />program. <br /> <br />operate this Adult Day Care program a minimum of five days per week and a minimum of six <br />hours per day, Monday through Friday, except for designated holidays, staff training days and <br />inclement weather conditions, in which most cases the Cabarrus County Schools policy will <br />be followed. ( Any additional closings must be pre-approved by the County. ) <br /> <br />8. assist the County with the final assessment and the development of the personal care plan of <br /> each client. <br /> <br />following the screening / intake process and an orientation visit, if enrollment into this <br />program is approved, a schedule will be established and the contractor will be eligible for <br />reimbursement beginning with the first full day of attendance. <br /> <br /> <br />
