FUlL <br /> <br />draft <br /> <br />This contract made this 30th day of May 1996 by and between Jon <br />Barry and Associates, Incorporated, hereinafter called the agency, <br />and Cabarrus County Ambulance Service, hereinafter called the <br />client. <br /> <br />witnesseth: <br /> <br />It is mutually agreed, understood and promised as follows: <br /> <br />Agency will useits best efforts to collect accounts for <br />the client. Agency shall not use threats, intimidation, <br />harassment, or violate any federal or state laws <br />regarding the collection of past due accounts. <br /> <br />Agency will remit to the client a monthly etatement oi <br />the gross amount received less agency fee. <br /> <br />Agency shall charge a commission o~ thirt¥-£ive percent <br />(35%) on all monies collected on all accounts. <br /> <br />Agency shall m~intain company records as they pertain to <br />said accounts, in such'a manner as to be audited by the <br />client at any time during normal business hours. Agency <br />will comply with maintaining account records as guided by <br />ctient, federal, or state regulations or policy, to make <br />available any records or files ~or seven (7) years from <br />date of placement. <br /> <br />Client shall report ail posted payments weekly to agency <br />if such payments are received by the client while the <br />account is assigned to the agency. <br /> <br />Contract may be terminated at the option of either party <br />by written notice given at least thirty (30) days prior <br />to the date of termination. <br /> <br />Agency will incur normal business expenses for any for~ <br />and equipment, including revision~ requested by client <br />personnel. <br /> <br />In witness whereof, the parties have executed this contact in <br />duplicate of the date above written. <br /> <br />Client: <br /> <br />Administrative Approvall <br /> <br />Title. <br /> <br />Agency: Jon ~arry end,Associates, Incorporated <br /> <br />By: .
