The purpose of this grant is to expand our Maternal Care Coordination
program. We wilt receive $1~),750 for April-June 1997. We plan to increase our staff by 2.5
positions resulting in an increased caseload of 125 clients by June 30, 1997.

This grant will provide start-up funding for two registered nurses and one part time social worker.
All of these positions will be revenue generating positions, with Medicaid as the third party payor
source. The start-up funds will provide for salary and training costs until the new staffmembers
have established a client base which results in sufficient Medicaid reimbursement to cover program
costs.

I am requesting your approval to add these new positions to the Matemal Care Coordination. Team.
These staff will provide case management to pregnant women during their pregnancy and 60 days
post delivery. This program has proven to provide a positive impact on the state Infant Mortality
Rate, Low Birth Weight rates, and the "No Prenatal Care" rates.

I am requesting these 2.5 positions be designated as follows:

(2) Full-time Public Health Nurses
(1) Part-time Social Worker

58-30-101
58-30-102

cc: Capitola Stanley
