Clifton, Jr, Cabarrus County Manager and Commissioners <br />From: Thelma Freeze, Executive Director, L.I.F.E. CENTERS, INC. <br /> <br />SUBJECT: REQUEST FOR MATCHING FUNDS ($3670.) REQUIRED TO ACCESS THE <br />ADDITIONAL $33,031.00 (NC State monies) ALLOCATED BY THE HCCBG COMMITFEE. <br /> <br />Per your request in letter dated October 9, 1996, listed below are the services that will be <br />provided by the grant funds allocated for the provision of adult daycare services in Cabarrus <br />County during the fiscal year 96-97. <br /> <br />The additional allocation plus the current request will purchase 1747 units (days) of service. <br />(L.I.F.E. Center is reimbursed $21.00 per unit of service via HCCBG.) One "unit of <br />service" provided by the L.I.F.E. CENTERS, INC. can include but is not limited to the <br />following: <br />-Transportation to and from the centers and local and extended field trips <br />-Breakfast, a hot balanced lunch, afternoon snack and occasional party refreshments. <br />-Two pre planned, structured programs daily in each site...to meet the needs of participants. <br />---All therapeutic, most are provided by community volunteers and staff_some professionals. <br />-Movement therapy, occupational, and music therapy and reminiscence therapy. Counseling, - <br />physical therapy and memory classes are provided to the participants by professionals <br />-Counseling, support groups and respite are provided for the caregivers. <br /> <br />These services are provided week days between 7:30 a.m and 5:30 p.m. by dedicated, caring <br />staff and volunteers. Hours and services will be expanded when the program relocates. <br /> <br />You are encouraged to compare the services provided in Adult Day Services to any or all of the <br />other services provided, hour for hour, service for service, to understand the cost-effectiveness. <br />This level of care has prevented or delayed institutionalization for hundreds of our local older <br />adults with physical and memory impairments .... a tax savings for all...and an improved quality <br />of life for both participants and their family members. <br /> <br />In view of the cost-effectiveness of this service, we hope you will agree to provide these <br />matching funds through the local lead agency, DOA, as you do for all of the other services <br />provided under this grant. The money saved in administrative fees to process the request each <br />year would surely cover the total cost to the county and guarantee the provision of these priority <br />services to this group of older adults with special needs. <br /> <br />Please advise if you need additional information; also, please advise if L.I.F.E CENTER staff <br />should be in attendance at this meeting to personally present this request. <br /> <br /> <br />
