This grant is funded through the Kate B.
Reynolds Health Care Trust Fund. This project is a joint effort of the Division
of Maternal and Child Health, The Division of Medical Assistance, and the Office
of Rural Health and Resource Development.

The Maternal Outreach Project is designed as an enhancement of the Baby Love
Program through the addition of Maternal Outreach Workers. Through this grant,
we have been awarded two positions for Maternal Outreach Workers, who will
provide services to a select population of pregnant women before their 28th week
of pregnancy and continue to support the woman and her family until the infant
is one year old. The Maternal Outreach Workers will work closely with the
Maternity Care Coordinators and the Child Service Coordinators to enhance the
care given to Medicaid eligible women in Cabarrus County who are at risk for poor
pregnancy and parenting outcomes.
