Howes, Secretary <br /> <br />I )FHNR <br /> <br />MEMORANDUM <br /> <br />TO: Local Health Directors <br />FROM: <br /> <br />January 6, 1995 <br /> <br />Annette Byrd, R.N., M.P.H., Chief <br />Immunization Section <br /> <br /> RECEIVED <br /> <br /> JAN 1 7 1994 <br />Ans'd <br /> <br />SUBJECT: Federal Immunization Action Plan (IAP) Funds <br /> <br />The Centers for Disease Control and Prevention (CDC) has awarded the North Carolina Immunization Section <br />$1,704,943 in IAP funds for calendar year 1995. Your county's IAP award is based on the renegotiatext aid-to- <br />counties equity formula that was approved by the Local Health Director's Association in 1994. Your county's <br />new federal IAP award is listed below: <br /> C~BARRUS <br /> <br />New IAP Funds: $21,278 <br />50% of new award: $10,639 <br /> <br />One-half (50%) of your 1995 county award is now available. Please prepare a budget revision that reflects this <br />award. The remainder of your award will be available on July 1, 1995. Return your budget sheet to John~ie <br />Robinson in DEHNR Purchase and Contracts by February 17, 1995. If you have not yet submitted your <br />budget page for the state IAP award allocated on October 28, 1994, you may submit uno page with both that <br />award and this new federal IA.P award. Counties who wish to utilize IAP funds to purchase any piece of <br />equipment with a unit value of $500 or more must obtain written permission from the Immunization Section. A <br />memo which includes the specifications of requested equipment should accompany the budget revision, and a <br />copy should be sent to your Regional Immunization Consultant. <br /> <br />Please remind staff who prepare your budget revisions and Local Expenditure Reports that the contract number <br />for IAP funds has changed from "4524" to "5715.' This change took effect when the Immunization Section <br />became part of the Division of Maternal and Child Health. This change should be reflected on all future budget <br />revisions and Local Expenditure Reports. <br /> <br />Please contact your Regional Immunization Consultant for on-site assistance with your lAP budget, or Paul J. <br />Garrison or Marybeth Lister at (919) 733-7752 if you have questions about this award.
