Page 2 <br /> <br />SiX MONTHS MEASURABLE OBJECTIVES STATUS <br /> <br />(Continuation Programs Only) <br /> <br />In the space provided, please list each measurable objective in your current program <br />agreement and indicate the degree to which your program has been successful in achieving <br />them. (Use data from first six months of the fiscal year.) <br /> <br />1) 100% of clients will improve school attendance (to every day) as documented <br /> by school notes and regular communication with school officials. <br /> <br /> We had two Cabarrus County boys enrolled in our program during the first six months <br />of this fiscal year. Of these, 100% improved their school attendance to every day. They also <br />evidenced an improved attitude toward school and verbalized an increased desire to continue <br />and graduate from school. One of the boys was also involved in extracurricular activities such <br />as SGA and football. <br /> <br />2) 75% of clients will improve school performance from failing grades to at least a <br /> C average, as evidenced by daily school notes and report cards. <br /> <br /> 100% of the clients were able to improve their grades to at least a C average. This was <br />accomplished through tutoring and close communication with the client's teachers. <br /> <br />3) <br /> <br />Delinquent and/or inappropriate behaviors will decrease by 80% as evidenced <br />by no new court referrals, positive home visits, and daily progress notes by Resident <br />Counselors. <br /> <br /> Both of the boys were able to achieve this goal. One boy was recently discharged from <br />our program back to his home in Cabarrus Co. His behavior has improved and he is currently <br />living with his mother. He has returned to public school and has successfully completed his <br />probation through Juvenile Services. Follow-up with the mother reveals that the client <br />continues to do well at school and at home. He is involved with follow-up services and <br />substance abuse counseling through Piedmont Area Mental Health. <br /> The second client continues in the program to date. He has achieved many of his <br />goals and is making progress towards graduation. He also has successfully completed his <br />probation and was recently notified of his release from supervised probation. <br /> <br />4) <br /> <br />Social and communication skills will improve by 50% through use of role play, <br />counseling, family meetings, outside activities, and positive reinforcement from <br />Resident Counselors. Evidenced by positive home notes, school notes, and <br />interpersonal relations at the group home. <br /> <br /> Social skills for 100% of the boys improved dramatically. They have learned to control <br />their anger and express their feelings verbally. Impulse control has also improved as <br />evidenced by an increased ability to discuss frustrations versus their initial desire to explode <br />verbally or physically. This has been noted both at the group home and by the parents during <br />weekend visitations. <br /> <br /> <br />
