Page 4 <br /> <br />Measurable Objective(s): Of the youth served: <br /> 1 ) 100% will show improvement in school attendance (to daily) as attested <br /> to by daily school notes. <br /> 2) 75% will show an improvement in academics (from failing to passing <br /> grades) within 2 grading periods as attested to by progress reports, <br /> report cards, and school conferences. <br /> 3) 80% will show a decrease in delinquent and inappropriate behaviors, <br /> as evidenced by abiding by group home, school, and parental rules, and no <br /> new court referrals. Documented by evaluation meetings, school <br /> conferences, weekly reports by Resident Counselors and weekend reports <br /> from parents. <br /> 4) 50% will improve social and communication skills through use of <br /> counseling, family meetings, outside activities, and positive reinforcement <br /> from Resident Counselors.
