Page 3B <br /> <br />60~ of all youths served by the program will be prepared for <br />long-term placement within 90 days of the entry date <br />(preparation is defined as stabilization of the child's <br />behavior to a point where he is read for placement and <br />development of a long-term care plan). <br />Of the children completing the 90 day program, 50~ will <br />achieve successful adjustment for six months following long <br />term placement. (Success shall be defined as: [1] The child <br />does not come to the attention of the Juvenile Court~ [2] The <br />chiid~ the social ~orker, and his legal guardian agree that <br />placement was successful six months after it was made.) <br /> <br /> Information Maintained for Effectiveness Measurement [continued]: <br />2. Department of Social Services will keep a folder on each child <br /> containing court order showing custody with Department of <br /> Social Services, termination summary on the child, case <br /> dictation involving family counseling and coordination of <br /> services to the child and family, monthly progress reports <br /> submitted by the home to DSS~ client tracking forms suDmitted <br /> to CBA office. <br />3.
