Non-Emergency Franchise <br /> <br />2. Ronald Frazier and <br />4217 Linwood Road <br />Gastonia, NC 28056 <br /> <br />Gene Helms <br />7500 Riding Trail Road <br />Charlotte, NC 28212 <br /> <br />3. Carolina Sports Medic <br /> EMT Sports Standby and Non-Emergency Transport <br /> <br />Mr. Ronald Frazier has served twelve years and continues to serve as a volunteer for <br />Dallas Rescue Services. In addition, Mr. Frazier recently served as a volunteer for <br />Gaston Life Saving Crew for two years. <br /> <br />Mr. Gene Helms was employed with Charlotte-Mecklenburg Medic for two and one- <br />half years and has also been recently associated with Gaston Life Saving Crew. Mr. <br />Helms is currently serving with Idlewild Fire and Rescue Department, located on <br />Idlewiid Road. <br /> <br />5. According to the state of North Carolina, a certification of franchise must be provided <br /> before state certification of ambulances can be provided. <br /> <br />6. A copy of state certification for each employee is attached. <br /> <br />EMT Stand-by and Convalescence Transport <br /> <br /> <br />
