METIIOI)OLOGY TO ADDRESS SERVICE NEEDS OF <br /> LOW-INCOME MINORITY ELDERLY <br /> <br />OBJECTIVE I: To ensure th(.-, delivery o[ s(;rvices is provided to thc idcntitied low- <br />income minority elderly in Cabarrus County. <br /> <br /> Cabarrus County Department of Aging provides congregate meals, <br />transportation and in-home aide services to all older adults requesting services <br />regardless of race, sex or economic status. The department makes particular efforts <br />to assure that the low-income minority elderly are served through the folk)winK <br />strategies: <br /> <br />A. Maintain statistics on Iow-income minority eldcrly <br /> clients enrolled in all service programs. <br /> <br />B. Continue outreach in low income minoeity communities. <br /> <br />C. Provide evaluation tools to evaluate program services and collect input to make <br /> improvements in providing services. <br /> <br />OBJECTIVI,: II: Maintain awareness for special needs off the Iow-income minority <br />clde~ly. <br /> <br />A. Maintain a cooperative relationship with h)cal minority organizations, <br /> businesses, churches, etc. in identifying those we arc not serving. <br /> <br />B. Continue to appoint minority individuals to serve on thc Community Care <br /> Block Grant Advisory Committee. <br /> <br />Main rain congregate mealsitcs in minority communities of Concord and <br />Kannapolis and continue to provide nutritional education programs, health <br />screening services and informational programs of other ~;ervic~s available. <br />Continue providing transportation to the congregate sites as w('ll as <br />transportation to medical/doctor appointments, human st~rvice ag(reties, <br />senior center activities and shopping. <br /> <br /> <br />
