Some cases may be <br />carried for a period of time following intensive treatment in order to <br />allow for follow-up monitoring of the youth's behaviors and the <br />families' functioning. Furthermore, this program may have a positive <br />influence on the younger siblings of the identified youth and thus have <br />an even greater long-term effect. <br /> <br />Youth referred are between the ages of ten to seventeen and must have <br />at least one parent, guardian, or custodian who will agree in ~iting <br />to participate in the Home-Based Program. Referral forms and <br />procedures have been developed with the Juvenile Court staff. A social <br />history is required from the referring agency. <br /> <br />MEASURABLE OBJECTIVES: <br />1. Continue to develop a service plan for 100% of the children in <br /> the program. <br />2. Reduce out-of-home placements so that 60% of the youth served by <br /> this program during the year will be in the home at the end of <br /> treatment. <br />3. Continue to reduce youth involvement in the juvenile justice <br /> system by 60%. <br />4. Continue to increase school attendance for youth. <br />5. Decrease school discipline problems while the youth is receiving <br /> services in the program. <br />6. Continue to reduce recidivism back into the Juvenile justice <br /> system for 60% of youth following termination with the service <br /> (Promotion of family self-sufficiency). <br /> <br />INFORMATION MAINTAINED FOR EFFECTIVENESS MEASUREMENT: <br />The program maintains budgetary information regarding revenues and <br />expenditures of CBA funds. Documentation of staff time involved in the <br />program is maintained through the Department of Social Services <br />Information System. Client files contain the following information: <br />social history from referring agency, referral form, application for <br />services, services plan, and case dictation. CBA client tracking forms <br />are also contained in client file and are submitted to CBA office. In <br />1990-91 we worked with the Bring It All Back Home Program (BIABH) from <br />Appalachian State University. They have supplied our staff with training <br />on the use of several forms and materials they developed to measure program <br />effectiveness. <br /> <br /> <br />
