Position Title <br />Supervisor <br /> <br />Basic Function <br />of Position <br /> <br />Major Areas of <br />Responsibility <br /> <br />Requirements: <br /> <br />HOMELESS PREVENTION WORKER <br /> Position Description <br /> <br />Homeless Prevention Worker <br /> <br />Executive Director, Cooperative Christian <br />Ministry in cooperation with Cabarrus County <br />Homeless Implementation Committee. <br /> <br />To provide homeless persons and families with a <br />quality case management process; to coordinate, <br />link and network services. <br /> <br />Case Management Program development and <br />supervision of process. <br />Study, plan, evaluate, refine and maintain <br />case management. <br />Maintain up-to-date client files. <br />Analyze client data and prepare quarterly <br />gaps-in-services reports. <br />Analyze client data to determine need for <br />Case Management. <br />Implement and maintain computerized Case <br />Management system. <br /> <br />II. Interagency Relations <br /> Serve as a liason to community agencies. <br /> Maintain contact with agencies regularly to <br /> serve as contact persons in service areas. <br /> <br />III. Case Advocacy/Referral <br /> Provide professional problem <br /> assement/solving and advocacy work to <br /> identified clients. <br /> Appropriately respond to incoming requests <br /> and follow-up when indicated. <br /> Assist with walk-in clients. <br /> Assist with opening and closing cases to <br /> assure linkage with appropriate services. <br /> Responsible for establishing a service <br /> plan, including continuity for interagency/ <br /> multi-agency service involvement, for each <br /> client. <br /> <br />IV. Other duties as assigned. <br /> <br />Bachelor's Degree in Social Work or related <br />fields. Three years' experience in direct <br />service delivery management. Strong knowledge <br />and skills in client advocacy. Good oral and <br />written communication skills. Ability to work <br />with the general public, agency professionals <br />and volunteers. <br /> <br /> <br />
