rage 2 <br /> <br />SIX MONTHS MEASURABLE OBJECTIVES STATUS <br /> <br />(Continuation Programs Only) <br /> <br />In the space provided, please list each measurable objective in your current <br />program agreement and indicate the degree to which your program has been <br />successful in achieving them. (Use data from the first six months of <br />the fiscal year.) <br /> <br />Measurable objectives met July 1, 1991 thru December 31, 1991. <br /> <br />1. 85% referred children served <br /> 100% - all 12 children referred were served <br /> <br />~p% prepared for long term pla.ge~en.t <br />75% - 9 of 12 returned home, placed in foster home or group home or <br /> placed with relatives as determined to be the best long term <br /> placement. <br /> 2 went to psychiatric hospital <br /> 1 terminated because of runaway. <br /> <br />50% achieved successful adjustment for 6 months <br />63% - 10 of 16 children placed between March 1, 1991 and September 30, <br /> 1991 have had no new court referrals and have made successful <br /> long term care placements. Two of the 16 children were terminated <br /> to training Schools and 4 have had continuous runaways or <br /> placements from group homes into psychiatric hospitals and back <br /> to group homes. <br /> <br /> Between 7/1/90 and 6/30/91, we had 21 children placed in the group <br />home; 13 were for less than 30 days, 3 for less than 60 days, 1 for less <br />than 90 days, and 4 for the maximum 90 days. If we have 5 children stay <br />for 90 days our maximum is 20 children served. <br /> <br />From 7/1/91 thru 3/31/92, we have had 18 children placed. Currently <br />there are 5 children placed in the group home as of 3/31/92. <br /> <br /> In the CBA 1st quarter report of FY-91, our statistics for the past 12 <br />months showed 66% referred by court, 88% referred as delinquent or status <br />offenders. Our performance indicators showed 62% decrease in additional <br />court referrals, 67% decrease in runaways, and 100% decrease in school <br />suspension/expulsion. 94% were between the ages of 14 and 17. 72% were <br />~ite, 27% Black. 77% female, 22% male. 79% were active with Juvenile <br />Court, 5% were deferred from Juvenile Court to DSS for placement and 16% <br />were at risk of Juvenile Court involvement. 42% had successful completion <br />of the program, 36% terminated because of runaway, 10% court terminated <br />placement, and 10% were placed in psychiatric hospitals. At time of <br />discharge 94% were enrolled in school, 42% had had court referrals while <br />placed at the group home 52% had had at least one runaway and 15% had been <br />in secure custody. At discharge 20% went to relatives or back home, 20% <br />went to group home or foster home, 10% went to training school, and 15% <br />were placed in hospitals. <br /> <br /> Last year was a difficult year for the group home children. We saw <br />many severe behavior problem youth requiring psychiatric treatment and <br />hospitalization as well as the number of children in runaway status and in <br />secure custody. Five children were certified "Willie M" at the t~me of <br /> <br /> <br />
