(xiii) Requirements for prior notification and approval <br />by the Executive Director of any new introduction of <br />wastewater pollutants or of any significant change in the <br />volume or character of the wastewater prior to <br />introduction in the system. <br /> <br />(xiv) Requirements for the prior notification and <br />approval by the Executive Director or his designee of any <br />change in the manufacturing and/or pretreatment process <br />used by the permittee. <br /> <br />(xv) Requirements for immediate notification of <br />excessive, accidental, or slug discharges, or any <br />discharge which could cause any problems to the system. <br /> <br />(xvi) A statement that compliance with permit <br />does not relieve the permittee of <br />responsibility for compliance with all <br />applicable Federal and State pretreatment <br />standards, including those which become <br />effective during the terms of the permit. <br /> <br />(xvii) Other conditions as deemed aDpropriate by the <br />Executive Director to ensure compliance with this <br />ordinance, and State and Federal laws, rules, and <br />regulaticns; tke term cf the permit. <br /> <br />-- (j) Permits Duration <br /> <br />Permits shall be issued for a specified time period not to exceed <br />five (5) years. A permit may be issued for a period less than a <br />year'or may be stated to expire on a specific date. <br /> <br />(k) DischarGe Reports <br /> <br />(1) Every significant industrial user or nondomestic <br />discharger collecting self-monitoring samples or splitting <br />samples for analysis shall file a discharge report quarterly <br />or at such intervals as designated by the Executive Director. <br />The Executive Director may require any other industrial users <br />discharging or proposing to discharge into the treatment <br />system to file such periodic reports. <br /> <br />(2) The discharge report shall include, but in the discretion <br />of the Executive Director shall not be limited to, nature of <br />process, volume, rates of flow, mass emission rate, production <br />quantities, hours of operation, concentrations of controlled <br />pollutants or other information which relates to the <br />generation of waste. In addition to the discharge report, the <br />Executive Director may require information in the form of <br />industrial discharge permit application or any other <br />information as deemed necessary. <br /> <br />37 <br /> <br /> <br />
