(B) Drilling fluids and additives; and, <br /> (C) any formation material from the racer-bearing <br /> zones or other source that enter the veil <br /> rosy <br />as a result o~ pumping at a rate vithin the <br />limits of the reported yield. <br />Well Identification Plate <br />(1) An identification plate shoving the drilling <br /> contractor and his registration number shall be <br /> installed by the veil contractor on the veil vtthtn <br /> 24 hours after the drilling is complete. <br />(2) The identification plate shall be constructed of a <br /> durable veatherproof, rustproof metal or equivalen£ <br /> material. <br />(3) The identification plate Shall he securely attached <br /> to the yell casing or enclosure floor around the <br /> casing where it is readily visible. <br />(1) The identification plate shall not be removed from <br /> the veil by any person. <br />(5) The identification tag shall be stamped vith a <br /> permanent marking vithin thirty (30) days after <br /> drilling is completed to ahoy the: <br /> (A) total depth of well, <br /> (B) casing depth (ft.) and inside diameter (in.), <br /> (C) screened intervals of screened yells, <br /> (D) gravel interval of gravel packed yells. <br /> (£) yield or specific capacity expressed in gallons <br /> per minute (gpm), or gallons per minute per <br /> foot of draw do~n (Epm/£t.-dd), and, <br /> (¥) static rater level and dace measured. <br />Well-Head Completion and Equipment <br />(1) The top of the casing shall be cut off smooth and <br /> level~ be free from dents and cracks, and shall <br /> terminate at least six inches above the concrete <br /> slab around the casing. <br />(2) The identification plate, if removed or obscured <br /> during pump installation shall be relocated and <br /> securely attached to the veil casing or enclosure <br /> around the casing by the person lnstsllint the pump <br /> and pumping equipment. <br />(3) All piping~ viring, and vents shall enter the veil <br /> at least eight inches above land surface. <br />(4) The veil-head shall be completely enclosed except on <br /> yells rich turbine or other type pumps and pumping <br /> equipment designed for all weather conditions. The <br /> enclosure shall be vell drained and constructed to <br /> prevent the entrance of surface and rain vater, <br /> insects and rodents, and to protect the veil-head <br /> equipment against freezing. The ~ell shall have a <br /> concrete slab vith a thickness of at least four <br /> ' inches and eitending a minimum of 2 feet outvard <br /> from the outer veil casing. <br />(5) Every yell shall be equipped by the person <br /> completing the veil-head v/th a usable access port. <br /> <br /> <br />
