Any and all expenses incurred by the Center for the provision of the <br /> Program. <br /> <br /> 6. POPULATI0~ TO BE SERVED. <br /> <br /> Any resident of Cabarrus County who: <br /> I a. is 60 years of age and older and/or their spouse and <br /> b. raqulrls constant 24-hour supervision. <br /> c. cannot be left alone eithe~ because of memory impairment, physical <br /> i immobility, or other problems that render them unsafe. <br /> There shall be no discrimination with regard to race, sex, creed or <br /> I national origin, tdmission for respite care will be based on caregiver's <br /> need and the appropriateness of the services offered by the Center to meet <br /> these needs. Priority is given to serving individuals with the greatest <br /> economic or social need wit~ particular attention to low income minority <br /> I individuals who the above <br /> <br /> 7. PROCEDURES FOR REFERRAL EROM PURCHASER. <br />I a. All referrals for the direct service provision shall be referred to the <br /> Department of Agini. Each case will be evaluated by the Purchaser and <br />I referred to the Center for the purchase of the service. <br /> b. Caregiver~s requesting respite care rill have an initial conference <br /> with the Center at least seven (7) days prior to admission. <br />I c. The Center will be responsible for explanation of policies and <br /> procedures to caregiver~s during conference. <br />I 8. The Center eRrees to meet with Purchaser one timt per quarter {o~ ~ore <br /> frequently, if requested) to evaluate respite service program operation and <br />Iquality of service, Heatings shall be scheduled at the convenience of the <br />Purchaser and the Center. Ail units of service shall meet the required <br />minimum service standards as adopted and implemented by North Carolina <br />Division of Aging. <br />! <br />! <br /> - <br /> <br /> <br />
