Archer, Aging Program Administrator <br />DATE: June 14, 1991 <br />RE: 'F¥'92 Contracts <br />Enclosed please find two copies of the Title III and/or Title <br />contract(s) that Centralina will execute with your Agency. These <br />documents are the foundation of our working relatiohship for the <br />1992 fiscal year. While these contracts do not include every <br />operating policy and procedure, they do include very important <br />details concerning the expected levels of service to be provided, <br />grant funds available and the method of payment for services <br />provided. Please review the entire document. <br />Payment of funds under any contract(s) is subject to the receipt of <br />the Notification of Grant Award (NGA) and subsequent funds from the <br />Division of Aging. As discussed at the June 6, 1991 contractors' <br />meeting, new allocations of Title III and other state funds are <br />expected from the Division of Aging during late June. Therefore, <br />any part(s) of the Title III contract with grant fund amounts are <br />stamped "DRAST." Revised pages will be inserted by Area Agency on <br />Aging staff following the new allocations and your Agency copy of <br />this contract will be forwarded to you at that time. No changes in <br />Title U allocations are anticipated. <br />Do not make any changes to the contract(s). If you have questions <br />about any part(s) of the contract(s), please call. To execute the <br />contracts, please have the authorized individual sign Page 12 of 12 <br />[Title III) or Page 8 of 8 (Title ¥) and ~ake sure that someone <br />attests the s~qnature in the blank provided to the left. Obtain <br />signatures in black ink if possible.
