CABARRUS COUNTY AMEND~EN~ TO CONTRACT <br /> <br /> This amendment, entered into this sixteenth day of July, 1991, by and <br /> between Cabarrus County Department of Aging (hereinafter called the <br /> "Purchaser") and First Assembly Adult Day Care for institutional respite care <br /> services (hereinafter called the "Center"). <br /> <br /> WITNESSETB TEL~T: <br /> <br /> WH]~EAS, the Purchaser and the Center into that certain agreement dated <br />July 1, 1991 pursuant to which the Center was to provide day care services <br />for those persons who qualify for institutional respite care services as <br />defined in the contract, and <br /> <br /> W~, certain changes need to be made in the implementation date of <br />the new service standards ~ndated by the Centralina Council of Governments <br />and the number of units to be purchased; <br /> <br /> NOW, THEPJ~'OP~E. it is agreed that the aforesaid contract be and it is <br />hereby emended in the followinE respects. Item number one (1) is emended by <br />inserting November 30, 1991 as the last date current standards are to be <br />provided. On December 1, 1991 the Center agrees to provide services and <br />activities in accordance to the new standards as provided by N.C. Division of <br />Aging. (Attached). <br /> <br /> Item number three (3) is amended by deleting unit purchase of up to 576 <br />unit hours with no more than &8 hours per client per month and in inserting <br />in lieu thereof units purchased based on availability of funds. <br /> <br /> <br />
