C/~,~dlItUS COI.1NTY l~--2~F.~r TO CONTRACT <br /> <br /> This amendment, entered into this sixteenth day of July, 1991, by and <br /> <br />between Cabarrus County Department of Aging (hereinafter called the <br />"Purchaser") and Giles Leisure ~ome for institutional respite care services <br />(hereinafter called the "Cente~"). <br /> <br /> WIT~SSETH THAT: <br /> <br /> ~II~EA$, the Purchaser and the Center into that certain agreement dated <br />July 1, 1991 pursuant to which the Center was to provide day care services <br />for those persons who qualify for institutional respite care services as <br />defined in the contract, and <br /> <br /> W~S, certain changes need to be made in the implementation date of <br />the new service standards mandated by the Centralina Council of Governments <br />and the number of units to be purchased; <br /> <br /> NOW, TH~ORE, it is agreed that the aforesaid contract be and it is <br />hereby amended in the following respects. Item number one (1) is amended by <br />inserting November 30, 1991 as the last date current standards are to be <br />provided. On December 1, 1991 the Center agrees to provide services and <br />activities in accordance to the new standards as provided by N.C. Division of <br />Aging. (Attached). <br /> <br /> Item number three (3) is amended by deleting unit purchase of up to 2& <br />days per client per contract year or up to 576 hours per client within <br />stated contract period m~d in inserting in lieu thereof units purchased based <br />on availability of funds within stated contract period. <br /> <br /> <br />
