CABARRUS COUNTY EMERGENCY MEDICAL SERVICES <br /> <br /> EMERGENCY MEDICAL TECHNICIAN STUDENT CLINICAL <br /> <br /> TRAINING PROGRAM <br /> <br />AGREEMENT made and dated this ~--~ day of <br />between Cabarrus Memorial Hospital , hereinafter referred to as <br />the Hospital, and Cabarrus County Emergency Medical Services, <br />Emergency Medical Technician Student Clinical Training Program, <br />hereinafter referred to as the Program. This contract supersedes <br />all previous contracts entered into by Cabarrus Memorial Hospital <br />and Cabarrus County Emergency Medical Services, Emergency Medical <br />Technician Student Clinical Program. <br /> <br />This agreement, reviewed annually, will be in effect from <br />~r~,~ ~, 19 gl , until terminated by either party by 90 days <br />written notice or revised by mutual agreement. <br /> <br /> I. The Hospital and Cabarrus County Emergency Medical <br /> Services hereby mutually agree with each other to the <br /> following: <br /> <br /> A.The Hospital is being designated as a resource for <br /> clinical experience in the program. <br /> <br /> B. The Hospital will be responsible for the <br /> organization, administration, operation, and funding <br /> of its services. It shall maintain standards <br /> accepted by the Joint Commission on Accreditation of <br /> Hospitals and shall retain responsibility for <br /> patient care. <br /> <br /> C. The Hospital, the Director of Cabarrus County <br /> Emergency Medical Services, and the on-site <br /> supervisor provided by the Hospital will cooperate <br /> in the evaluation of the program to include removal <br /> of participants whose performance is determined to <br /> be detrimental to patient well-being or achievement <br /> of the objectives of the Program. <br /> <br /> <br />
