Since July 1, 1989 thru April 10, 1990. 2{ children were served in the <br /> Lutheran Group Home, Fifteen of the.e youth were ceferred by juvenile court for <br /> <br /> out, run away, etc. ~t that poi~xc the tasks of reuniting the child with the famiXy may <br /> <br /> they are placed out of tt~e home. Leaving the child wigh 11o home to return to, resulting in <br /> <br />utilized prior co custody being placed with the Department of Social Service$ and <br /> <br />insuring the "reasonable efforts" part of Public Law 96-272 (Adoption Assistance and Child <br />Welfare Act of 1980). This act requires that cl{iid welfare agencies make "ceasonable <br />efforts" to prevent or eliminate the need for removal of a child from his home, and to <br />have implemented a pre-placement preventive se~ices program designed to help children <br />remain with their families. Ail service~ offered prior <o a child's placement in foster <br />care are evidence of compliancd with this mandate. ~ia must be done in order to ueceive <br />federal roarer care funds. <br /> <br />This program is staffed by a full-time social worker. Tills worker'~ s¢{iedule ia flexible <br />in that the worker may be making home visits and contacts with the youth and his family <br /> <br />Referrals to the program are in written fo~ and completed by the referring agency <br />assisted by the social worker. The referral is ceviewed by the supervisor of the <br /> <br />interview occurs.
