PROFESSIONAL SERVICES CONTRAC'~ <br /> <br /> This Contract, made and entered into this 1st day of June, 1990, <br /> by Cabarrus County Health Department and Management Recruiters/Sales <br /> Consultants of Concord, Inc. referred to as "Provider", a corporation <br /> with offices in Davidson and Concord, N.C. and represented by I~aRoy <br /> Deabler, M. II.A., <br /> <br /> That Nhereas, ~]e NortJ] Carolina General Statutes expressly allows <br />Ca~s County to appropriate funds for the pui~pose of facilitating <br />health services withJ n the said county; and <br /> <br /> Whereas, Provider has agreed to contract to provide professional <br />services in recruiting a qualified OB/Gyn dcoLor to work in the Cabarrus <br />County Health Department, Concord, N.C.; <br /> <br /> Now, Therefore in consideration of the premises set forth herein <br />and furtJ]er consideration of the terms and conditions set' forth herein.,. <br />the parties hereto covenant and agree as follows: ' <br /> <br /> 1. The Provider agrees to perform the following services: <br /> a. Conduc~ an extensive search for a qualified candidate for the <br /> OB/Gyn Position including COntact with approved residency <br /> training programs in 0B/Gin <br /> b. Screen candidates accroding to Health Department criteria <br /> c. Interview appropriate candidates and market Health Department <br /> position in most favorable terms <br /> d. Consult with Dr. Allen Dobson, Dr. Vincent Arey, and Fred <br /> Pilkington on specific requirements of position <br /> e. Provide in writing three (3) references on candidate <br /> f. Set up telephone and face-no-face interviews <br /> g- Guarantee qualified candidate no later than July, 1991 <br /> h. Provide progress reports on advertisements results, interviews <br /> and reference checks <br /> i. Periodically meet with Health Department officials to review <br /> program of recruitment. <br /> <br /> 2. The Cabarrus County Health Department agrees to the following: <br /> a. To pay Provider as follows: <br /> (1) $ 3,000.00 Retainer before July 1, ~990 <br /> (2) 6,000.00 Within one week after face-to-face interview <br /> (3) 9,000.00 Within one week of signing of employment contract <br /> by candidate and Cabarrus County Health department <br /> $18,000.00 Total Fee <br /> b. TO intea~iew any candidate presented by the Provider who meets <br /> desired qualifications so specified by the employer within 48 hours. <br /> c. ~b rei,~erse Provider on an hourly rate of $100.00 plus travel <br /> expenses not to exceed $2,500.00 per year for practice consultations <br /> and visits to candidates prior to hiring <br /> <br /> <br />
