CABARRUS COUNI'Y AM~h~D~D~NT TO CORTRACT <br /> <br /> This Amendment, entered into, this 13th day of Sap,em-oar , 1990, by and between <br />Cabarrus County Department of Aging (hereinafter called the "Purchaser'*) and First <br />Assembly Adult Day Care Center for adult day care services~(hereinafter called the <br />"Center"). <br /> <br /> WIT~ESflETH ~HAT: <br /> <br /> ~'HEREAS, the Purchaser and the Center into that certain agreement dated <br /> <br />July 1, 1990 pursuant to which the Center was tn provide day care services for <br />those persons who qualify for respite care as defined in the ccntract, and <br /> <br /> ~EREAS, certain changes need to be made in ~urchase and delivery price for <br />all units of aerviee ordered by the Purchaser; <br /> <br /> NOW, THEREFORE, it ia agreed that the aforesaid contract he and ~t is hereby <br />amended in the followimg respects: <br /> <br /> Item number five (5) is amended by deleting the unit price of $3.~ per ho~r <br />and inserting in lieu thereof the following unit pri~e of $18.00 per day (with a <br />si~ hour minimum stay) shall be the purchase and delivery price for all units of <br />service ordered by the Purchaser. Monthly request for payment by the C ~ter shall <br />include but not be limited to an individual participant summary ¢onsi~ting of the <br />dates,-.and total hours, not to exceed 42 hours per month and the total amount owed <br />by Purchaser. <br /> <br /> <br />
