APPENDIX H-2 <br /> <br /> CONTRACT FOR RESPIT~ CARE <br /> ADULT DAY HEALTH FACILITY <br /> <br /> T/IS AGREEH~NT, entered into as of this 1st day of . .Inly , by <br /> <br /> to as the "Center") and ~abarrus C~unty Department of Aging~ (hereinafter referred <br /> <br /> " ~ITI~ESS£TH THAT: <br /> <br /> ~EREAS, 'the Purchaser desires to engage the First Assembly Adult Day <br /> Health Center to render services in connection with the Respite Care : <br /> Program for the older adults of Cabarrus County (hereinafter referred to as <br /> the "Program")j described in the Purchaser's contract with Centralina <br /> Council of Governments designated for the provision of respite care <br /> services through Title III funds~ SSBG funds and Senate gill 1}59 funds to <br /> older adults provided to the Purchaser through Centralina Council of <br /> Governments from North Carolina Division of Aging and United States <br /> Department oi Health and Human Setwicesl and <br /> <br /> ~£RF~tSj First Assembly Adult Day Health Center is a licensed adult day <br /> health facility ~ith the demonstrated capacity and desire to render adult day <br /> health services to older adults; <br /> <br />KO~ THEREFORE, in consideration of the premises and the mutual covenants <br /> and agreements hereinafter contained, the parties hereto agree as follows: <br /> <br /> 1. The Center agrees to sell and Purchaser agrees to boy Center's respite <br /> services. The Center agrees to provide services and activities in <br /> accordance with standards as provided by ~.C- Division oi Aging. <br /> (Attached). The Center agrees nor to subcontract for the delivery of <br /> sex~ices called for in this agreement unless ~utually agreed by <br /> Purchaser and the Center. <br /> <br />2. Ibis agreement ~ill be in effect and services will be delivered <br /> beginning .l,ly 1. 1990 an~ continuing through .hm~ ~O. lqql <br /> <br />3. Purchaser will purchase up to 576 unit hours with no more than 48 <br /> hours per client per month within the stated contract period. The <br /> Center shall be co~itted to deliver the total units of services <br /> requested by the Purchaser. <br /> <br /> The Center will deliver the Program based on an one hour unit of <br /> service. Program will be provided in the Adult Day Health facility. <br /> <br />5. The purchass and delivery price for all units of service ordered by <br /> Purchaser shall not exceed a cost of $~6.00 per day. This is the <br /> Center's medicaid reimbursable rare.
