CABARRUS ARTS COUNCIL, INC. <br /> CONTRACT FOR 1990-91 N.C. GRASSROOTS ARTS PROGRAM GRANTS <br /> <br /> Acceptance of this grant constitutes 'an obligation upon the <br /> applicant to fulfill and comply with the following requirements <br /> during the grant period July 1, 1990- June 30, 1991. The signature <br /> of the applicant's authorizing official certifies that the <br /> applicant agrees to fully abide by the requirements set forth <br /> within this contract. <br /> <br /> Requirements: <br /> <br /> 1. The applicant must match this grant according to the project <br /> budget submitted within the grant application or any subsequent <br /> approved revision of that budget. <br /> <br /> 2. The applicant must ensure that N.C. Grassroots Arts Program <br /> funds are not used for the following: <br /> <br /> a) personnel positions, including but not limited to wages <br /> and benefits. <br /> b) deficit funding, including but not limited to expenses <br /> incurred prior to the grant period. <br /> c) capital expenditures, including but not limited to <br /> building, renovating, or remodeling facilities. <br /> <br /> 3. All published materials and announcements regarding the funded <br /> project should contain the following special note: <br /> <br /> "This project is supported by a grant from the Grassroots Arts <br /> Program of the North Carolina Arts Council, a state agency and the <br /> Cabarrus Arts Council, Inc." <br /> <br /> 4. The applicant must submit the grantee evaluatio~ form with any <br /> necessary supportive material to the Cabarrus Arts Council within <br /> 30 days of the completion of the funded project or by June 15, <br /> 1991, whichever comes f~rst. We will forward the evaluation forms <br /> upon receipt from the North Carolina Arts Council. Evaluation <br /> forms MUST be typed. Copies of programs, newsletters, press <br /> releases and any other published materials concerning the funded <br /> project to the Cabarrus Arts Council; P.O. Box 809; Concord, NC <br /> 28026-0809 must be included in the evaluation. <br /> <br /> 5. The grant recipients must complete an interim re~ort by January <br />· 30 describing the progress of the project. I the project has been <br /> completed, the. final evaluation form should be submitted instead of <br /> the interim report. See attached form. <br /> <br /> 6. All recipients must submit a written request to make any <br /> material changes to the scope of the project as described in the <br /> original application. Approval from the Cabarrus Arts CouncliMUST <br /> be made prior to incorporation of any changes. <br /> <br /> <br />
