CO. ,CT SSRVXCSS <br /> <br /> THIS AGREEMENT, entered into as of this 13th day of November, 1990 by <br /> and between Safeway Cab Company hereinafter referred to as the "Vendor" <br /> and Cabarrus County Department of Aging hereinafter referred to as the <br /> "P~rchaser." <br /> <br /> W I TN E S SETH THAT: <br /> <br /> WHEREAS, the Purchaser desires to engage the vendor to render <br /> services in connection w~th the transportation program (hereinafter <br /> referred to as the "Program"), for the older adults in Cabarrus County in <br /> the Purchaser's contract with the North Carolina Department of Public <br /> Transportation designated for the provision of transportation services <br /> through EHTAP funds. <br /> <br /> WHERP~S, the vendor is a licensed transportation service company <br /> with the demonstrated capacity and desire to render transportation service <br /> to older adults. <br /> <br /> NOW THEREFORE, in consideration of the premises and the mutual <br /> covenants and agreements hereinafter contained, the parties hereto agree <br /> as follows: <br /> <br />1. Vendor agrees to sell and purchaser agrees to buy on a limited basis <br /> transportation services for older adults in Kannapolis to the Royal <br /> Oaks Nutrition site for this program according to the provisions of <br /> the agreement. The vendor agrees not to sub-contract for the delivery <br /> of the services called for in this agreement. <br /> <br />2. This Agreement will be in effect and services will be delivered <br /> beginning November 14, 1990 on Wednesdays and continue through <br /> June 30, 1991 or until funds are exhausted. <br /> <br />3. The Purchaser will purchase up to 10 trips with no more than one trip <br /> to and from the nutrition site. The vendor shall be committed to <br /> deliver the total n%u~ber of trips requested by the purchaser. The <br /> Vendor shall deliver a trip based on $6.00 per trip for three clients <br /> in zone. Additional client's cost will depend upon the zone in which <br /> they live. <br /> <br />4. The Vendor will transport to Royal Oaks United Methodist Church <br /> wednesdays during the period of this agreement with the exception <br /> of the holidays. Pickup clients at 10:45 - 11:15 a.m. and deliver <br /> to nutrition site. Clients will be picked up at the nutrition site <br /> at 12:30 p.m. to be delivered home. <br /> <br /> Thanksgiving Day November 22 & 23, 1990 <br /> Christmas December 24-26, 1990 <br /> New Year's Day January 1, 1991 <br /> Memorial Day May 27, 1991 <br /> <br /> <br />
