Page I of 7 <br /> <br /> CONTRACT FOR RESPITE CARE <br /> TEMPORARY PLACEMENT <br /> <br />THIS AGREEMENT, entered into as of this day of 1989', <br />by and between Giles Leisure Home (hereinafter referred to as the Center) <br />and Cabarrus County Department of Aging (hereinafter referred tb as the <br />"Purchaser" ). <br /> <br /> WITNESSETH THAT: <br /> <br />W~I~REAS, THE Purchaser desires to engage the Center to render services <br />in cofinection with the Respite Care Program for the older adults <br />of CabarrusCounty (hereinafter referred to as "Program"), described <br />in the Purchaser's contract with Centralina Council of Governments <br />designated for the provision of respite care services through Title <br />III funds, SSBG fund and Senate Bill 1559 funds to older adults provided <br />to the Purchaser through Centralina Council of Governments from North Carolina <br />Division of Aging and United States Department of Health and Human <br />Services; and, <br /> <br />W~EREAS, The Center is a licensed domiciliary care service provider <br />with the demonstrated capacity and desire to render domiciliary services <br />to older adults; <br /> <br />HOW T~EREFORE, in consideration of the premises and the mutual covenants <br />and agreements hereinafter contained, the parties hereto agree as <br />follows: <br /> <br />1. The Center agrees to sell and Purchaser agrees to buy Center's <br /> respite services. The Center agrees to provide services act5vities <br /> in accordance with standards as provided by North Carolina Division <br /> of Aging. (Attached). The Center agrees not to sub-contract <br /> for the delivery of the services called for in this Agreement <br /> unless mutually agreed by Purchaser and The Center. <br /> <br />2. This Agreement will be in effect and services will be delivered <br /> beginning October, 1989 and continuing through .June 30, 1990. <br /> <br />3. Purchaser will purchase up to 24 days per client per contract <br /> year or up ~o 576 hours per client within the stated contract <br /> period. The Center shall be committed to deliver the total units <br /> of services requested by the Purchaser at an average minimum <br /> rate of 24-hour days or 168 hours per client. <br /> <br />4. The Center will deliver the Program based on one hour unit of <br /> service. Program will be provided in the domiciliary home. <br /> <br /> <br />
