AREA AGENCY ON AGING <br /> SERVICE SPECIFIC BID SPECIFICATIONS <br /> RESPITE <br /> BelOw are service specific bid specifications required by'the Area <br /> Agency on Aging in addition to (or to clarify) the North Carglina <br /> Division of Aging Service Standards. <br /> <br /> ~it of Service_t <br /> Since the unit of service is based on time, the documentation of <br /> units of service must include the time (11:00 - 12:00) along with <br /> allowable service activities. ~ <br /> <br /> TaraetGroUDt <br /> ~he Older 2tmerican'S Act emphasizes the socially needy and <br /> economically needy with special emphasis on low income <br /> minority elderly. North carolina is emphasizing rural elderly. <br /> These tarqet groups should be specifically identified, attempts <br /> to ser~e the group documented, and procedures be established for <br /> how the target group clients will be given priority for r~ceiving <br /> the services, clients served in this service must be designated in <br /> the Moderately ImpairRd or Frail elderly category. <br /> <br />~:'3. while all service activities are not required for each client, the <br />(" Agency should have allowable service activities available as <br /> needed. <br /> <br /> Deliver; charac~istic~i <br /> The Agency should have in each client file: <br /> Written assessment completed in the home within ten days of <br /> referral <br /> Written plan of care <br /> Documentation of quarterly monitoring contact <br /> Admlnistr~ti~ ~e~remen~s~- <br /> ~he Agency should have:. <br /> Documentation of completion of required training. <br /> Current job descriptions for each staff member <br /> written grievance brocedures for clients <br /> Procedures for collecting program income <br /> Procedures for evaluation of se~~vice by participants (at least <br /> annually) - <br /> Policy for termination of services <br /> Written plan for how the Agency will provide general and <br /> service specific information and procedures for referral <br /> Written plan and documentation of outreach activities <br /> <br /> <br />
