- 2- <br /> <br /> We would like to apply for the grant and receive the $6250 in start-up <br />funds in April. A Social Worker II would be hired to begin work by May I co do <br />community educations efforts, establish referral systems with other agencies, <br />work with service providers, and begin accepting client referrals for <br />screening. The Clerk Typist III would be needed June I to establish hilling <br />procedures for Medicaid reimbursement and provide clerical support to the CA~-DA <br />program. Expenses for thia period total $5831. <br /> <br /> For fiscal year 1987-88 total revenues of $79,330 are expected with $25,000 <br />from the LTC grant and the remainder from Medicaid reimbursement for services. <br />Expenses for services to clients, staff, and program administration total <br />$78,060. <br /> <br /> We feel this program could be a valuable community service to help support <br />family efforts to keep elderly, disabled persons at home for as long as <br />possible. <br /> <br /> <br />
