7. The first responder and the first responder agency shall <br /> be liable for their actions pertaining to the care of the <br /> patient prior to the arrival of the ambulance provider. <br /> <br />SECTION g: Standards for Record Keeping. <br /> <br /> I. Each first responder agency shall maintain the following <br /> records: <br /> <br /> A. Record of Patient Treatment: For each patient <br /> rendered assistance by the first responder agency, <br /> the following Information will be recorded on a <br /> patient treatment form: <br /> (I) Name, age, and address of the patient_ <br /> (2) Date and time treatment rendered. <br /> (3) Location of treatment rendered. <br /> (4) Patient complaints and condition at the time of <br /> treatment rendered. <br /> (5) Patient vital signs and time recorded. <br /> (6) Treatment rendered the patient by the first <br /> responder. <br /> (7) Name of the member of the ambulance provider to <br /> whom patient is relinquished. <br /> (B) Printed names and signatures of the first <br /> responders who provided patient care. <br /> 8. A preliminary copy of t~& record of Patient <br /> Treatment shall be given to the ambulance crew when <br /> he/she assumes control of the patient's care at the <br /> <br /> C. A copy of the completed Record of Patient Treatment <br /> will be given to the Director of Cabarrus County <br /> Emergency Services at any time prior to the <br /> quarterly meeting of the Cabarrus County EMS <br /> Advisory Board. This copy will be used for audit and <br /> <br /> D. The First Responder report will contain the dispatch <br /> number. The number will be the same as the EMS <br /> dispatch number. <br /> <br />SECTION 10: Grievance Procedure. <br /> <br /> 1. First responder agency will be responsible for <br /> resolving grievances between-its own members. <br /> <br /> 2. Grievances from a first responder agency toward Cabarrus <br /> County Ambulance Service shall be submitted in writing by <br /> the first responder and co-signed by the Fire Chief or <br /> his/her designee to the director of Cabarrus County <br /> Emergency Services. <br /> <br /> <br />
