A file is <br />maintained by Lutheran Family Services on each child admitted. This file contains the <br />following forms: <br /> <br />(1) Child's information sheet completed at aclmission~ and an ad~ission agreement. <br />(2) Placement Progress Report. This form keeps a running record we~kly of what progres~ <br /> has been made toward a plan for this child in leaving temporary shelter care. <br />(3) A weekend activity planning record. <br />(4) A daily log showing the child's disciplinary actions, DSS contact, personal observation <br /> by 5taf£ and Co~ents~regarcLin. E tho child's adjustment that ~lay., <br />(5) An approach plan which describes the child s problems, needs and approach to ~eet~nE <br /> the needs. <br />(6) A basic clothing list. <br />(?) Receipts for purchases <br />(8) A termination Sl,~rn~ry <br />(9) Con, ants from the staff or other information as needed. <br /> <br />At the DSS, information is kept in the child's folder regarding the date he was placed at <br />Lutheran Home, each conference that is held with the child or his family or any outside <br />agency, all medical, psychological and education conferences and a copy of /he termination <br />sun~aary completed by Lutheran Family Services. In addition, the CBA tracking forms are <br />kept on each child referred. Contracts for consulting services, copy of Jicense, names and <br />qualifications o~ individuals employed as staff and amount of training as well as proof of <br />availability of medical assistance is kept on file at Lutheran Services. Coordination with <br />law enforcement is done through the task £orce ~eetings as welY as the screening co~ittee <br />~eetings and averages contact at least monthly. A listing o£ the ~embers o£ the local adviso <br />board is kept by the task force chairperson. The CBA client tracking £orms will evaluate the <br />number of children referred and those who ~ere not admitted and ~'hy they were rejected. <br />Lutheran Homes will keep a file of the number o~ children who ran away from the project <br />facility during this period. Lutheran Homes will keep a log of who visi;s the residents and <br />dates o£ visits. <br /> <br />The DSS will continue to evaluate the number o£ children who are involved in a crisis <br />situation and compare that number wi~h the number of ad~isslons to the Temporary Shelter <br />Care. Statistics will be kept regarding number o£ children placed in other facilities who <br />were in crisis situations as well as the number placed in Lutheran Home. Lutheran Home <br />and DSS £iles ~ill provide in£or~ation to doctunent the number o£ children who ~ere placed <br />in the Temporary Shelter Care and those who achieve stability and those who did not. The <br />DSS will contact each child after he has completed his stay at the Temporary Shelter Care <br />six months after discharge date to determine if the child continueg to reside in placement, <br />if he has come to the attention o£ the cot~rt, what his school adjustment has been and i£ <br />the child's parents, guardian or placement person agrees that the situation has been stable <br />for the past six months. This will be done by a contact from DSS. The CBA Evaluation <br />Co~ittee will ~onitor goal achievement through on-site visits, client tracking forms, and <br />review. <br /> <br /> <br />
