Page'~ a ..... <br /> <br /> GOAL OF PROGRA~i: To provide during a 12 month period, shelter care facilities ~or up to <br /> 30 children (depending on length of stay) to meet immediate separation <br /> from their families. The shelter care facility will serve as an alternative <br /> to and diversion from present or future involvement in the criminal <br /> justice system and/or detention. ~e ultimate goal sill be to reduce <br /> the incidents of juvenile crime in the community. <br /> <br /> STATEMENT OF ~IEASURABLE OBJECTIVES: <br /> 1. Provide an emergency residence for 75 percent of all children <br /> determined by service workers to need such care (5 children will be <br /> cared for at one time and 20 will be cared for on an annual basis). <br /> 2. For 60 per cent of all children who enter the program, prepare for <br /> long-term placement within 90 days of tile entry date (preparation <br /> is defined as stabilization of tile child's behavior to a point where <br /> he is read), for placement and development of a long~terntare plan - <br /> preferrably in the child's o~m hone). <br />3. Family Counseling for 50 per cent of the residents to enhmice <br /> parent/child relationships in an effort to underscore greater <br /> possibility of success in preparation for long-term placement in <br /> tile home. <br /> <br /> II~FOP3~TION b~INTAINED FOR EFFECTIYEN~SS MEASURE~IFJ~: <br /> The Cabarrus County Department of Social Services will keep a listing <br /> of all residents of the Tenrporary Shelter Care including placement date, <br /> discharge date, behavior descriptions, ffequency and type of court <br /> involvement, after care placements, school achievement and attendance, <br /> run-away behavior, cost of care and evaluations by DSS staff of tile <br /> effectiveness of the placement, as well as CBA client tracking forms. <br /> <br /> In addition, Lutheran Family Services will keep a file nn each child to <br /> monitor his-placement at the Shelter Care and will prepare a behavior/ <br /> attitude summary on each-resident following termination of placement. <br /> For each resident who stays long enough, Lutheran Family Services will <br /> fornulate a Plan of Care stipulating Purpose of Placement and including <br /> time oriented goals. The Department of Social Sen, ices' staff will make <br /> contact with tile child and/or his family six months after the child <br /> leaves the Shelter Care to determine if the child bas'~dhieved stabili~" ' <br /> in his long-terra placement according to the measurable objectives as <br /> stated above. <br /> <br /> <br />
