ATTACHMENT A= <br /> <br />STATEMENT OF M~A$~RABLE OBJECTIVES= <br /> <br />1. 75% of the identif~ed students for individual and group <br /> therapeutic counseling will complete the program. <br /> <br />2. 85% of the identified students for therapeutic camplng/field trips <br /> will complete specified activities. <br /> <br />3. 75% of students who complete the therapeutic counseling/field trip <br /> activities/learning experiences will show improved positive <br /> self-concept as measured by a self-concept test. <br /> <br />4. 50% of those who have completed the group counseling program and <br /> who have had a history of office visits for disciplinary reasons, <br /> truancy, suspension, expulsions, and/or court referrals will have <br /> shown a reduction in these referrals during the courts of the <br /> counseling/activities experience. <br /> <br />ATTACHMENT B: <br /> <br />INFORMATION MAINTAINED FOR EFFECTIVENESS MEASUREMENT: <br /> <br />1. Successful completion of t~e program will be measured by item 13 <br /> of the CBA Client Tracking Form for School Related Programs. <br /> <br />2. Behavioral attitudinal checklist will be completed by a classroom <br /> teacher at the conclusion of the group and individual counseling <br /> experience as shown by item 15. <br /> <br />3. Ail students, upon entering the therapeutic counseling program, <br /> will be administered a self-concept test such as Pierce-Harris <br /> Self-Concept Scale. <br /> <br /> Each student, as he completes the counseling experiences, will be <br /> administered the same test as on entering the program. <br /> <br />4. Hased on information obtained from school records and/or <br /> interviews with the school guidance counselor, principal, teacher, <br /> student, agency referral sources, data will be gathered concerning <br /> office visits for disciplinary reasons, truancy, suspensions, <br /> expulsions, and court referrals. <br /> <br /> <br />
