SAMPLE <br /> PROCLAMATION <br /> <br /> WHEREAS, in a time of escalating cost of medical care <br />in hospitals and nursing homes, more attention is being <br />placed on home health care; and <br /> <br /> WHEP~EAS, provision of special follow-up care can mean <br />earlier discharge from the hospital for many; and <br /> <br /> WHEREAS, through teaching in the home setcing~ the <br />patient and family learn to attain maximum independence; <br />and <br /> <br /> %C4EREAS, patients are usually happier at home in a <br />familiar place where the patient feels secure and content- <br />ment usually resuits in a better recovery; and <br /> <br /> kMEREAS, home health agencies provide skilled nursing <br />care and other health services under a physician's super- <br />vision and may eliminate the need for mere costly long <br />term institutional care; now <br /> <br /> THERFORE, I, (name) , ( ti tle) <br />do hereby proclaim the week of November 25, through <br />December 1, 1984, as, <br /> <br /> hO.~ CARE WEEK <br /> <br />in (county or city) , and encourage all citizens to <br />recognize the importance of horm~ health care being provided <br />to many elderly and disabled individuals in their homes, <br />no; only by professionals, but also by their families and <br />loved ones. <br /> <br /> <br />
