BLUE CROSS BLUE SHIELD HOSPITALIZATION INsuRANcE <br /> <br /> RETIRED EMPLOYEES <br /> <br />Retired employees under the age of 65 will remain under our group <br />coverage at the same premium rate as active employees. <br /> <br />Retired employees ranging from the ages of 62 and 65 will effect <br />our experience modification because these claims will be charged <br />to our group. <br /> <br />Individual coverage for retired employees 65 and older is $ 31.95 <br />monthly. The employee may obtain spouse coverage at the same <br />monthly premium of $ 31.95 if the spouse is age 65 or older. <br /> <br />Retired employees 65 and older will not effect our experience <br />modification because these claims will not be charged to our grouP. <br />They are charged to the regular B C B S non group coverage. <br /> <br />*This addition is subject to the approval of B C B S underwriting <br />division. <br /> <br /> <br />
