NORTH C~ROLINA CONqL~,ACT ~OR <br /> ~CKLENBURG COUNTY ~RGENCY SERVICES EQUIP~T <br /> <br /> THIS AGREF2~tT, made gad entered £nto this day of , 1982, <br /> by gad between Cabarrus Coun.t¥ , hereinafter referred to as the <br />County, gad the CENTRALINA COUNCIL OF GOVE-~N}~NTS, hereinafter referred to as the <br />Council; <br /> <br /> WITNESSETH: <br /> <br /> WHEREAS, Cabarrus CouaEy desires [o improve their emergency <br />medical services program, and has submitted to the Council a proposal for such <br />improvements; and <br /> <br /> ~{EREAS, said proposal was included in the E~ Area !II Emer~emcy Medical <br />Services System Imp. rovement - 1204(1) application prepared by the Council ~or <br />submittal to the U.S. Department of Health gad Human Se_~vices through the N.C. <br />Department o~ Human Resources; and <br /> <br /> Wq{EREAS, the Council's application, including the County's proposal, has <br />been approved and subsequently funded; <br /> <br /> NOW T~EREFORE, in consideration of the premises and payment of the funds <br />as hereinafter set forth, both the ~County and the Council do mutually, agree to <br />the following: <br /> <br /> GENERAL U~OERSTJJ~INGS <br /> <br /> 1. The Council agrees to provide or to cause to be provided those certain <br /> items of ~-MS Communications Equipment sat forth ~u E~uibit A, attached <br /> 'z hereto and incorporated herein by reference. <br /> <br /> 2. The Council agrees to pay 75% of the ~otal purchase price of those <br /> certain items set forth in Exhibit A, provided that under no circum- <br /> stances shall the =-mount paid by the Council exceed $ S1%4~026,6~ <br /> <br /> 3. The County agrees to provide toward the total purchase price ga amount <br /> equal to at least 25% of the total purchase price of items se~ forth <br /> in Exhibit A. <br /> <br /> 4. Payment of funds by ~he Council as specified in this agreement is <br /> conditioned upon gad subject to the. Council's receipt of funds allocated <br /> and appropriated for this purpose from the N.C. Department of Human <br /> <br /> 5. The County' agrees to name as their <br /> official project director for all matters pertaining to this agreement. <br /> <br /> 6. ~ne County agrees to submit in w~iting to the Council any reqJest fo~ <br /> changes in the aforesaid equipment or budget therefor, and to receive <br /> the approval of the Council prior to im?lementing any such changes. <br /> <br /> <br />
