Students will remain in the progrsam tu~til successful completion or until <br />terminated- upon the recommendation of the co~u~.selor/teacher with the consensus <br />of the school-based committee. <br /> <br /> Follow-up eye, ration of this project's success will involve completion of the <br />CBA Client Tracking Form for the school related programs, and a teacher-completed <br />questionnaire designed to assess classroom behavior. A pre and post progrs_m <br />evaluation of self-concept using a recognized instrument such as the Pierce- <br />Harris Self-Concept Scale or the TeD_uessee Self-Concept Test will also be <br />accomplished. Relative referral rates for mai-adaptive behavior before entering <br />and upon completion of the progrsun will also serve as another indicator for <br />program success. <br /> <br /> It should be stated that at this writing the position has not been filled. <br />~Lirin~ %,ril! be accomplished by a committee composed of representatives from Concord <br />City School S~stem and %he Cor~.~unity Based Alternatives Task Force. Upon fil~ing <br />the position~ certain chanfes may be necessary within the pro§ram format.
