The questionnaire <br />LENGTH will be completed via a personal interview. <br />CONFiDENTiALiTY' - Precautions have been taken to protect the confiden- <br /> tiality of any information provided all respondents. <br /> The survey form will be identified only by a number~ <br /> and the respondent~s name will not appear on any of <br /> the survey reports. <br /> <br /> <br />
