TRUSTEES

,,^,,,uo.,o, 'N. C. FIREMEN'S PENSIO FUND OEUU'SU.~OU,.EE

May 15, 1978

TO: ALL RATED AND CERTIFIED FIRE DEPARTMENT

Enclosed are two '~nnual Certification of Firemen" forms.

Each fire department must attach s roster of its active firemen to this
annual certification form. Then the certification form must be completed
by the fire department's Governing Body. The Governing Body of a municipal
department is the Town Board or Council. When a fire department serves a
fi~e district, two certifications and two rosters must be returned, one for
the fire department and one for the fire district.

North Carolina General Statutes, Chapter 118-23 requires that every fire
department having members in the North Carolina Firemen's Pension Fund must
have their Governing Body submit to this office annually a certification of
their firemen.

Even though your fire department may not have members participating in the
N~ C. Firemen's Pension Fund, your Governing Body must submit an annual certi-
fication with roster attached of all active firemen to this office. This is
required in order to maintain the line of duty death benefit provided by the
State for firemen.

FAILSRE TO RETURN THIS CERTIFICATION AND ROSTER INCURS THE LOSS OF ELIGI-
BILITY OF MEMBERSHIP IN THE N. C. FIREMEN'S PENSION FUND AND ALSO THE LINE
OF DUTY DEATH BENEFIT.

Thank you for your cooperation in this matter, I am

Sincerely yours~

Clara S.
