154 <br /> <br />DEFINITIONS <br />Hospice patient: An individual who has been certified as terminally <br />ill by their physician and is receiving care from Hospice of <br />Cabarrus County, Inc. <br />Attending physician: A doctor of medicine who is identified by the <br />individual, at the time he or she chooses to receive Hospice care, <br />as having the most significant role in determination and delivery <br />of the individual's medical care. <br />Hospice employee: A person, working under the jurisdiction of the <br />Hospice, either salaried or volunteer. <br />Patient Care Coordinator: A registered nurse designated by the <br />Hospice to coordinate the implementation of the care plan for each <br />Hospice patient. <br />Hospice Interdisciplinary team~ A group of Hospice employees and <br />volunteers, consisting at a minimum of a doctor of medicine, a <br />registered nurse and a social worker who provide or supervise the <br />care and services offered by the Hospice. <br />Care Plan: A written plan prepared for each Hospice patient, <br />containing an assessment of the Hospice patient's needs, <br />identification of services to be provided, and a detailed <br />description of the scope and frequency of services needed to meet <br />the patient's and family's needs. <br />SERVICES <br />Hospice will provide 24 hour, seven day a week on-call hospice care <br />to selected patients and their families utilizing a multi- <br />disciplinary team approach. Hospice care is palliative rather than <br />curative, and Hospice seeks to meet its patient's physical, <br />psychological, social, and spiritual needs in a manner that promotes <br />the Hospice philosophy of affirming life. <br />COMPENSATION <br />County and Health Department shall pay Hospice $5,592.00 for <br />rendering 200 hospice patient care days to appropriate residents of <br />Cabarrus County within the county fiscal year 1989-90. <br />TERM <br />This Agreement shall be in effect from July 1, 1989, to June 30, <br /> <br />1990. <br />AGREED TO THIS DATE: July 1, 1989 <br />CABARRUS COUNTY <br />By /s/ James W.
